For your safety and the safety of patients, all volunteer blood donors must be evaluated to find out if they are eligible to give blood. Completing the donor questionnaire is your first step.

If there is a question you do not understand, or want to discuss eligibility based on your responses, contact 1 888 2 DONATE (1-888-236-6283). This could prevent an unnecessary visit to the blood donor clinic. Please note that final eligibility is determined by staff at the blood donor clinic.

Remember to answer all questions truthfully. To make a false statement is a serious matter and could harm others.

Questionnaire responses are time-sensitive and are only valid on the day of your visit. Your responses to this questionnaire will be encoded on a Q-pass. Print your Q-pass or send it to your mobile device and bring it with you.

Closing the questionnaire before printing or saving your Q-pass on your mobile device will cause your responses to be lost. If printing, do not close your browser window until you are sure that your Q-pass has printed.
